To begin we will need a referral from your family doctor.

Our new patient coordinator will be in contact with you within 1 week after we receive the referral. We look forward to meeting with you!
Your First Consultation:
- Both partners (when applicable) need to come to the first consultation.
- Please complete your Health History Forms in advance of your appointment. This can be found as a downloadable PDF at the top of this page.
- All partners need to present their OHIP cards at reception for all appointments.
- Please come 15-20 minutes before your appointment time to ensure proper patient registration. You can expect to be at the clinic for up to two hours.
- During your 1st consultation you will meet with a Clinic Nurse as well as our Medical Director. They will address all your questions and concerns.
- You can expect to have additional tests ordered.
- We will likely want to schedule a follow up appointment for approximately 6 weeks from the consultation.
The clinic is located at 4271 King Street East in Suite 200 in the Deer Ridge Centre. There is lots of free parking available. We recommend entering from Deer Ridge Drive. Please arrive at least 15-20 minutes early to allow for proper registration and to allow us to review your completed Health History Form.
We are in a scent-free building.

All our patients are important to us and we want you to have as much time as you need so you can understand your complete care needs. Because everyone is unique we may without notice need more time with some patients than others. We appreciate your patience when these circumstances arise.
